All team members must fill out a Mercedes Benz Cotton Row Run individual Entry Form. The team coordinator must submit individual entry forms with fees of $15 each if postmarked by March 31 and $20 each if postmarked between April 1 and May 19, along with this entry form and a $25 Corporate Cup entry fee to the address at the bottom of the page. All Corporate Cup entries must be postmarked by May 19, 2011 to be officially entered. No changes can be made after May 19.
